The Aktilite CL128 lamp with light-emitting diodes (LEDs) is intended for use in photodynamic therapy in combination with Metvix (methyl aminolevulinate) cream 160mg/g. The LED array is placed in a rectangular pattern and is coupled to a matching array of lenses that produce a collimated red light beam. The lamp emits a light of high uniformity with a narrow spectrum at approximately 630nm, which can be adjusted by a control panel. The light dose used is 37 J/cm2 and is combined with Metvix cream. After topical application of 5-aminolevulinic acid, protoporphyrin IX (PPIX) accumulates intracellularly in the treated actinic keratosis lesions. The intracellular PPIX is a photoactive, fluorescing compound and, upon light activation in the presence of oxygen, singlet oxygen is formed which causes damage to cellular compartments of the light-exposed target cells, in particular the mitochondria.

The pharmacological action of retinoids may be explained by their effects on cell proliferation, cell differentiation, apoptosis, angiogenesis, keratinisation, sebum secretion and immunomodulation. Unlike other retinoids, which are specific agonists of either RAR or RXR receptors, alitretinoin binds to members of both receptor families.
